If you are interested in filling out an application with our agency, you are welcome to come to our office located at 1419 K Street in downtown Modesto, CA. We can also complete applications via email, fax, or by printing the forms on our forms tab on this website. Please just give us a call at 209-549-2954 and we can facilitate the application process for you.

Please note:

*If the client is in a board and care or nursing home, we will need a copy of the admissions agreement.

*If the client is renting their own apartment/house/trailer, we will need a copy of the rental agreement.

*If the client is renting a room only, we have a form that must be signed by both the renter and the landlord.

*If the client is renting from a relative, we have two forms that must be complete by both the renter and the relative (we require you to bring a copy of the rental agreement and all utility bills).

*If the client is homeless, we have a form that must be signed directly by the client.

We ask that if you have it, please bring: State issued ID card, Medi-Cal/Medicare card, EBT card, or other form of ID. Thank you so much for your time and business, we look forward to helping you in the future. :)
